Man, hilarious story. Sorry you had such a rough time. My experience with the dentist is quite the opposite of yours in that I haven't had any bad experiences. They always say my teeth are too perfect.

The last dentist I went to in the UK was a Scottish woman who specialised in dentistry-phobic patients. She was absolutely great! And there was no visible cleavage :tu

Don't worry about dignity, I'm sure they have seen (or are going to see) much worse than this!
But yeah, if I didn't want to get caught loking and drooling (which I don't) I'd close my eyes. It doesn't show that you're terrified - especially if you tell them that you are just going to relax a bit while they do their job... But you'll have to ease up on the clenched fingers, lol

Last time I went to the dentist, I ended the day in the ER for severe pain and swelling in my jaw. The jerk forgot my requirement to NOT get a mandibular block (like you describe in your original post) because it can cause bleeding issues for me. Well....he gave me one anyway.
Thankfully my girlfriend is really caring and was able to take care of me that night. That sucked.
