I have never drilled solid epoxy and don't know how hard that would be but my concern would be how to clamp the rubber doll and not damage it while doing so. I would rather form the hole when the epoxy was liquid.

Now you have my mind wandering again. If you got a pipe cap. Probably plastic. Or a plug used to keep the end of valves or pipes etc clean until use that had an inside diameter of the outside of the knot. Glue that with epoxy inside of the head at the proper depth. Then you could shape and round, polish the epoxy and top of head to what you want before gluing the knot in. If you wanted to cover it up you could use the brass ring, or else leave it visible and work it into your design.

I might have to take up the Troll Challenge! LOL

Why don't you want a troll brush?
I think we should have a novelty brush club. I guess the woodworkers amongst us could just carve a troll and make it into a brush, this is really just meant to be a cheap fun project.
I think a wobble head might make a good brush handle too.......
