The hands that are holding the (not so) private part(s) have been extremely modified. Notice the left index finger has been amputated. The guy must be into harvesting his own extremities.

As for the brush:

I note that it was invented in Scotland, perhaps when you wear a kilt and nothing else you pick up some excess dust and need to sweep more often?
