collision location/time/description
would this be sufficient ? (maybe a lawyer lurking?)
Do yourself a HUGE favor and get a proper release written by a lawyer. There are much more expensive matters to consider than the damage. The release has to be for all current and future claims against you, the company, anyone else in the vehicle (hers and yours), for future damage, medical issues etc... I guess you get my point. Just my 2cents.
