Wifey bought me a 100mm bottle of David Craig Bay Rum for Christmas. On the label it says "to be used as a fragrance in lotions and colognes."
Does this mean it should be added to something, or diluted with something, or should I just slap it on as is?

A little googling didn't bring me many answers, but it looks to be some kind of hospital aftershave. I would assume as with any aftershave that it is meant to be used straight. Why don't you just give it a shot and see if its similart to other aftershaves?

Biotechpharma lists that product under "skin care, ready to use". If there is a chance it is undiluted perfume oil, do not use it before checking, as undiluted perfume oils are very strong and can be damaging to the skin, they can even cause rather nasty side effects. It should say clearly in the ingredients list or in the instructions if it is indeed ready for use on skin.

If it is a fragrance oil, do not apply it straight to your skin. Mix it in an aftershave or make your own using artisan's alcohol.
