    I think it is a pick but for cleaning under fingernails, not for teeth.

    I have a similar set that has that item but is missing the small pair of scissors. The rest of the set is genuine tortoiseshell but the pick I feel is artificial, perhaps some sort of plastic. It has definitely been colored with the dark spots to look like tortoiseshell, it doesn't feel like smooth tortoiseshell but is slightly gritty or rough, and there is a seam down the length along the side.
