Unfortunately I cannot tell you make and model. I can tell you that it was a package deal. It was when I first started shaving, before I found SRP. It could have been a e-shave package or some other brand. I am not sure. I am sure that every once in awhile everyone has a bad apple.
I do not submerge the whole brush in the water, but when I say warm, it is steaming. I figure just from the 10 minutes in the shower with all the steam it was bound to happen after a week or two.
Since joining SPR I haven't had that problem, but I know now to pay attention to all the stuff that happens so I can share the good and the bad with everyone.

It sounds like the same problem I had with a package deal from the "non-LLC" site that sells $50 vintage "damascas steel" razor (shaped objects). The brush I got from them that had the same problem appears to have a VDH logo on it. (Not saying it is VDH made, just that the logo is on it). I temp-fixed mine with some rubber cement -- seems to be holding now.
