You wouldn't cut yourself due to the same factors that prevent a butterfly knife from cutting you. When holding the proper handle, only the back of the blade (spine side) can hit your hand. You can tell which side to hold it by based on which side the clasp-thingy (yea, that's the technical term; you didn't know?) is on.

To answer the OP; I think it would be possible, but you would need to do something near the pivot hole because butterfly knives require two pivots on a section that is generally wider than the tang of a straight. I would think you could attach (weld?) tabs onto either side of the tang and put pivot holes into the tabs.
