For me, it's usually a state of mind. If I have a restless night or something is on my mind, I have one of those bad shave days. I was fully aware of this the last time it happened because it was difficult to keep my mind on the shave and focusing was a real effort.

Yeah, looking back on the shave, my mind wasn't totally there. I was kind of let down because I had been looking forward to my next shave for a couple days, and it turned out terrible. Oh well, maybe next time will be better

Distraction can be bad, no matter what your doing. It was 6 months ago, I got into an argument with some guy at work, and it was bothering me. As I was stropping my blade, my mind began to re run the situation over in my head. My hand decided to flip prematurely, and slice my Long English Bridle strop beyond repair. Long story short, it became a paddle, and a hard lesson learned for me.

When you are shaving with a straight or even honing or stropping you need to be in a mellow mood. Any kind of agitation or issue that distracts you even on the subconscious level can lead to a bad outcome.

Maybe we should include meditation as part of the pre shave ritual eh?

Isn't that one of the big reasons we all have stuck with straights instead of the quick and convenient razors everyone else uses? It's relaxing and is our own way of meditating. But I do agree that some pre-shave meditation would be a good start.
